New Client Forms
For your convenience, I have made the new client forms available for download. If you would like, you can print and complete these forms and bring them in with you to your first session. If you have any questions about these forms we can discuss them at your first visit or feel free to email me. If you are coming in for couples counseling, please make sure both you and your spouse complete two copies of the "New Client Information" form.
